Assessment applicants must provide their assessor with their completed training record (logbook).  This logbook must show...

• the name and address of the trainee.
• the name and address of each employer for whom the trainee does work.
• the name and signature of each person who supervises the trainee while the trainee is doing that work.
• a description of the work as it is actually done by the trainee in the course of training.
• a record of the date on which the work was done.

If the applicant cannot provide an adequate record of training, the assessor must not conduct the assessment, as this is a prerequisite for trainees.  (Guidelines for Certificate Assessors.  Guideline 25 )

 

Before undergoing any Workcover Licence Assessment personal identification totalling 100 points must be provided to your assessor. This identification must be original and certified photo copies will not be accepted. The following table shows acceptable I.D. and the point value for each form of I.D.
